Mission Statement

Clinton County Adult Day Center, Inc. offers a community based program that provides supervision and or assistance with the activities of daily living to impaired adults in a protective group setting. This service enables clients to remain in the community, and allows families to continue caring for their loved ones at home, thus avoiding premature or inappropriate institutionalization.

Goals:

  1. Promote the individual's maximum level of functioning for as long as possible.
  2. Promote emotional, physical, and spiritual well being.
  3. Provide respite, support, and education for the families and other caregivers.
  4. Maintain the client in the community for as long as possible, avoiding premature or inappropriate institutionalization.
  5. Serve as an integral part of the community service network in the continuum of care.

Services:

  1. Care for adults who may need some help or supervision with activities of daily living.
  2. Assistance for people who cannot manage all of the tasks of daily living but do not need placement in long term care facilities.
  3. Provides programs for those recovering from stroke or surgery, or those suffering from Parkinson's or other diseases.
  4. Provides clients with activities and a place for socialization.
  5. Provides structured activities and individual care to help people with Alzheimer's and other memory impairments to stay involved with everyday life.
  6. Provides people with healthy balanced meals and nutritious snacks.

Interdisciplinary Team Approach:

The Adult Day Center Staff will work with the client and or family and the client's physician to prepare an individual plan of care.

Depending on the person's needs and the services offered it may include:

  1. Health screening - tests to identify and prevent health problems.
  2. Meals - Hot meals, snacks and or specialized diets, depending on nutritional needs.
  3. Recreation - dancing, games, tournaments, crafts and more.
  4. Exercise - scheduled walks, aerobics, sports and dance.
  5. Transportation - a ride to and from the Center.
  6. Counseling - for clients and families as needed.
  7. Cultural activities - movies, speakers, music and dance.
  8. Education - programs that encourage learning new information or skills.
  9. Reality orientation - help with remembering the basics of every day life.
  10. Medication assessment - a detailed look at all medication for drug interactions and other adverse reactions.
